Benefit Description: PacifiCare HMO--No deductible, $30 office visit co-pays, $500 hospitalization co-pay (services rendered at SVDH-zero cost), $100 emergency room visits. PacifiCare PPO--$500 deductible, 80/20 with participating provider, 60/40 with non-participating provider, Pre-tax dollars, $15 co-payment
Prescription Drug Insurance
Eligibility: Enrolled in health insurance.
Benefit Description: Services through local participating pharmacies and mail order. HMO--$10 generic, $20 brand name, $25 non-formulary. PPO--$10 generic, $20 brand name, $35 non-formulary.
Dental Insurance
Eligibility: Full-time & part-time employees and their dependents.
Premiums: Employee paid.
Benefit Description: Participating dentists only. $50 deductible/ 100% preventative services covered. 80% basic services, and 50% major or orthodontia. Orthodontia coverage available for dependent children only. 12 month waiting period on major and orthodontic treatment without previous dental coverage. Pre-tax dollars.
Vision Insurance
Eligibility: Full-time & part-time employees and their dependents.
Premiums: Employee paid.
Benefit Description: $25 annual co-payment. $105 paid towards contact lenses. Frames covered in full every two years. Pre-tax dollars.
Life Insurance
Eligibility: Full-time & part-time employees and their dependents.
Premiums: Employer paid.
Benefit Description: $10,000 life insurance policy for employee only (reduced to $3,500 at age 65). Must be enrolled in hospital's health insurance.

401(a) Profit and Pension Trust

457 Deferred Compensation

Benefit Description: A service award program is held annually to recognize employees for years of service. Service awards are given for each five year anniversary.
