Incident (s):
- Alaris pump may give fail alert with the following error #25516275
Contributory Factors:
- May happen when IV tubing is loaded quickly into channel.
Steps to take:
- Please notify Biomed at ext. 4764
- Remove pump from room, apply red tag, and do not use until Biomed has cleared pump for use.
