CalAIM Initiative Helps Homeless!

Sierra View Medical Center participates in the new CalAIM initiative by helping those with managed Medi-Cal plans access these new services. Two programs, now in place; Extended Care Management and Medically Tailored Meals are there to help Medi-Cal recipients to access both housing assistance and nutritional food.
