Looking to Obtain a Copy of Your Medical Records?

Important Notice: Because access to your medical information is important and essential, we will be available to process your requests in person at our Medical Records Office or we can process your requests for release of information remotely. If you have any COVID symptoms or would like service without entering our office, please call us at (559) 791-4714. As we make an effort to protect our patients and employees from the COVID-19 virus, please be advised that you will be screened for COVID-19 upon entering our office and face coverings are mandatory.

Paper Copy of Medical Records

Federal HIPPA laws require an authorization to be completed for all medical record requests, unless your provider is requesting them for your care. Because your medical record and the information it contains is confidential, we must confirm your identity before releasing your medical record. Only the patient or his/her legal guardian and/or legal representative can sign the form. You can download the authorization form below.

Walk-In Hours

To obtain your medical records, please bring your valid photo ID or two of the documents listed below with you to the Medical Records department at 396 W. Putnam. We are available to accept medical record requests between the hours of 8 a.m. and 5 p.m. Monday through Friday. You can also fax the request forms with a legible copy of your ID to our fax number at (559) 791- 4826 for processing. We will call you when your records are ready to be picked up.

  • Authorization for Release of Protected Health Information (PHI) Form completed and signed or
  • A current government issued photo ID (passport, driver’s license, State ID, or military ID) and
  • Power of attorney for healthcare paperwork if the patient’s POA is requesting records

Please be aware it may take up to 10 days to receive your records.

Our goal is to get your health information to the appropriate health care provider as quickly as possible; therefore, we recommend that your physician (or other healthcare provider) request a copy of your medical records to be delivered directly to his/her office by fax.

Patient Portal

The Patient Portal is your own personal portal where your health information is in one secure, easy-to-use online patient portal including easy access to your medical records. It gives you access to most of your medical records on your desktop computer, laptop, tablet or smartphone 24 hours a day. Advance Directives

An advance health care directive lets your physician, family, and friends know your health care preferences, including the types of special treatment you want or don't want at the end of life, your desire for diagnostic testing, surgical procedures, cardiopulmonary resuscitation and organ donation. The form below can help you create your own advance health care directive.
