Participate in a Biometric Screening and Get Rewarded

You are eligible for a confidential biometric screening as part of the Sierra View Medical Center’s Health Insurance Biometric Program.

Many serious health conditions develop over time. They may be delayed or prevented if you know your risks and make healthy changes. A biometric health screening may help you better understand your health numbers and what they may mean to your overall health.

Learn Your Numbers For:

• Total cholesterol
• Blood pressure
• Blood sugar (glucose)
• Body mass index
  
  When/Where:
• Feb 23, from 7a.m.-1p.m. – Langley/Russell Room
• Feb 24, from 12p.m.-6p.m. – Langley/Russell Room

As part of the UHC RALLY Program, you will receive a $75 gift card of your choosing for attending!

New this year: Your spouse is also able to attend, free of charge as long as they are on your health insurance.

Get Started by Registering!

• Visit https://www.myuhc.com/ and log in.
• Click on the link ”Health Resources" within your dashboard.
• Scroll down to the Programs section and select the Get Screened program tile.
• You will arrive at the Quest Diagnostics site. Complete the screens that follow to finish the registration process.
• Once you’ve registered, you’ll arrive at the dashboard.
• Under the Biometric Screening section, select how you would like to participate and follow the instructions to schedule your appointment. We highly encourage you to choose the on-site event, which is the most convenient method.
  
  You should receive a "Results are ready" email when your results are available online. Sierra View Medical Center will not have access to your individual results.
