Onboarding Appointment
HR should have emailed you an itinerary of the date, time and location
of this appointment.
This appointment will include
- CLINIC - Pre-employment Physical Exam
- EMPLOYEE HEALTH - A drug screen (if your job requires one). TB test (if
a chest x-ray is necessary due to a prior positive TB test, please allow
an additional hour to complete the test. If a chest x-ray was done within
the last 3 months, please bring documentation with you). Immunization screening
- HR - You will have a photo taken for your SVMC badge. As such, please dress
in professional attire (i.e., scrub uniforms, business attire). You will
meet with your department Leader to go over what to expect on your first
day and your initial work schedule.
Please follow dress code standards for your onboarding appointment.
What to Bring

Current Driver’s License or photo identification is required for
a drug screening (if your job requires one). Tip: Please hydrate in preparation for urine drug screen and potential
lab draws.
- Eye Glasses if needed
- Immunization/titers records.
- License, Certifications, Provider Cards (Must be American Heart Association),
and degree if required for your position.

I-9 acceptable and unexpired documents establishing your identity and authorization
to work in the United States. You may present one selection from List A or a combination of one selection
from List B and one selection of List C.
